Service Description

YouCraft Wellness provides personalized wellness coaching to support individuals and families in their pursuit of improved health and well-being. The wellness coach, offers a tailored approach to help clients define and achieve their wellness goals through supportive, fun, and engaging coaching sessions.

Individual Wellness Coaching: Personalized sessions designed to help clients identify their goals, address challenges, and develop sustainable habits.

Family Wellness Coaching: Coaching sessions that focus on strengthening communication and relationships within families, with an emphasis on collective well-being.

Holistic Approach: An integrated focus on physical, mental, and emotional health, tailored to fit the individual needs and lifestyle of each client.

Referral Discounts

Discount Offer: For every client you refer, both you and the new client will receive a $15 discount on your next session.

Important Details: This discount is not cumulative. If you refer multiple clients, each $15 discount will apply to individual sessions and cannot be combined.

Sliding Scale

At YouCraft Wellness, we understand that wellness services should be accessible to everyone. We offer a sliding scale fee structure based on income for those who qualify.

Application: Interested individuals must complete a sliding scale application to determine eligibility.

Income-Based Fees: Discounts are provided based on the applicant’s income level.

Limited Availability: Please note that we have a limited number of slots available for sliding scale clients.

Payment Terms

Payment is due at the time of service. Clients are required to add a payment method on file prior to their first session.

Session by Session Clients: Payment will be processed after each session.

Monthly Clients: Clients will be billed monthly on the same date as their service start date. (e.g., if services begin on the 10th of a month, billing will occur on the 10th of each subsequent month.)

Accepted Payment Methods: Visa, Mastercard, American Express, Discover, JCB, and Diners Club.

Cancellation and Rescheduling Policy

Although YouCraft Wellness does not have a strict cancellation policy, advance notice is appreciated if a client is unable to attend their appointment. In cases of no call, no show, your coach will reach out via phone or text to confirm if the client still plans to attend. Clients are responsible for rescheduling any missed or canceled appointments.

Confidentiality and Privacy

Your privacy is our priority. All personal information and coaching session details are kept confidential and securely managed through SimplePractice, a HIPAA-compliant platform, ensuring your information is protected and handled with the utmost care.

Client Responsibilities

Clients are expected to attend sessions, communicate openly, and provide accurate information relevant to their wellness journey.

No Guarantees

While we strive to offer valuable support and guidance in your wellness journey, we recognize that individual results may vary. Each person’s experience is unique, and while we are dedicated to working together toward your goals, specific outcomes cannot be guaranteed.

Limitation of Liability

You acknowledge that our services are intended to provide guidance and support within a collaborative coaching process. While we are committed to assisting you in your wellness journey, we cannot be held liable for personal decisions made based on the coaching provided. Ultimately, the choices you make are yours to determine what best supports your goals and well-being.

Termination of Services

Clients may cancel services at any time. The coach reserves the right to terminate services if there are no further goals to pursue or if the client engages in disrespectful or unprofessional conduct. If such behavior continues after the coach has requested that it cease, services will be terminated.

Dispute Resolution

In the event of a dispute, open communication is encouraged to directly address any concerns. The coach is committed to finding a mutually agreeable solution. If needed, compromises will be sought to ensure a positive coaching experience for all parties involved.

Modification of Terms

We reserve the right to modify these terms at any time. Clients will be notified of significant changes.
